Steps for Food Establishment Applications:

1--Look through the following list to determine which category your application is under.  If you have previously had a food permit issued to you, use the "renewal" application.  If you are applying for a new permit or have changed ownership, use the "new" application. 

2--Click on the link and an application will be opened.  You must have the newest version of Adobe Reader to open the applications.  If you do not have this version, you can download a free copy at www.adobe.com.

3--Once the application is opened, type in the correct information and print a copy. 

4--Sign the printed copy and mail along with a check for the appropriate amount made out to Hancock County Health Department.
